Enrollment is available to individuals and families who are not already covered by a dental insurance carrier either through their employer or a self-covered plan.  Pre-existing conditions do not disqualify a person from membership in our program. This is NOT an insurance, so there are no waiting periods or annual maximums...just the work you need done when you need it!

Membership Annual Fees:

Individual: $99.00

Each Additional Family Member: $79.00

Your paid membership will be in effect for one full calendar year from the date of purchase.  This membership entitles you to a discounted schedule of fees for all treatment which is accepted and completed during that calendar year.  This Unique Smiles Discount Fee Schedule will save you between 15% and 40% of all services that are currently offered by our providers including preventative, diagnostic, periodontal, minor and major restorative, prosthodontics, endodontics, implants, orthodontics and all emergency treatment.
